• Your full name, date of birth, residential address, telephone number, and email address.
• Relevant medical history, medications, allergies, diagnostic imaging reports, and clinical assessment notes.
• Medicare, Private Health Insurance, Department of Veterans' Affairs (DVA), or NDIS details required for rebate processing and GP correspondence.

3. How Your Information Is Used

Your information is used strictly for your clinical assessment, treatment planning, appointment reminders, billing/rebate claims, and communication with your referring General Practitioner or allied health care team where clinically appropriate or required under Medicare EPC / DVA guidelines.

4. Secure Storage & Cliniko Practice Management

Patient records and online bookings are managed through encrypted healthcare practice management software (Cliniko). Access is restricted to authorised clinical and administrative personnel at Eternal Podiatry.
