Achilles Tendinopathy & Tendon Pain
Stiffness, thickening, or aching at the back of your heel or lower calf requires targeted tendon load management—not just passive rest.
Understanding Achilles Tendinopathy & Tendon Pain
Achilles tendinopathy is an overload injury of the body’s largest tendon, connecting the calf muscles to the heel bone. It typically presents either in the mid-portion of the tendon (2–6 cm above the heel) or at its insertion directly onto the back of the heel bone. Because tendons require progressive mechanical loading to remodel collagen fibres, complete rest often leaves the tendon weak and prone to flaring up as soon as you resume activity.
Common Symptoms & Presentation
- Morning stiffness and aching at the back of the ankle or heel during your first steps out of bed
- Pain at the start of a walk or run that warms up during activity, then aches more strongly the next day
- Localised tenderness, thickening, or a small nodule along the Achilles tendon
- Pain at the back of the heel bone irritated by firm shoe heel counters
Underlying Biomechanical & Clinical Causes
- Sudden spikes in running mileage, hill work, speed sessions, or court sports
- Reduced calf muscle strength and endurance relative to daily or sporting demands
- Subtalar joint overpronation creating a "whipping" torsional stress across the Achilles fibres
- Low-drop footwear transitions or stiff ankle joint mobility
How Eternal Podiatry Assesses This Condition
We differentiate mid-portion from insertional Achilles tendinopathy (as they require different exercise ranges), assess retrocalcaneal bursitis, measure single-leg calf capacity, and evaluate rearfoot mechanics during gait.
Evidence-Based Treatment Pathways
Tailored Tendon Loading Program
Isometric, heavy slow resistance, and eccentric calf strengthening tailored to whether your pain is mid-portion or insertional.
Learn moreRadial Shockwave Therapy (EMS)
Clinically supported shockwave sessions to reduce tendon pain and stimulate tissue remodelling alongside rehab.
Learn moreHeel Lifts & Custom Orthotics
Temporary heel elevation or custom 3D orthotics to reduce compressive and torsional strain on the tendon.
Learn moreFrequently Asked Questions
Why does stretching make my back-of-heel Achilles pain worse?
If you have insertional Achilles tendinopathy (where the tendon attaches to the heel bone), dropping your heel over a step compresses the irritated tendon against the heel bone. We modify your exercises to avoid painful compression.
Can I keep running or walking with Achilles tendinopathy?
In most cases, yes—using a structured pain-monitoring framework where we adjust speed, hills, and recovery days while building calf strength.
How many Shockwave Therapy sessions are usually recommended?
Clinical protocols typically involve 3 to 6 weekly sessions combined with a progressive calf loading program.
